No visa. Italy is an EU member state and Lithuania is in the EU and the Schengen Area, so Italian passport holders enjoy EU freedom of movement and can enter Lithuania visa-free with no time limit on short stays. Italian citizens do not need a visa to travel to Lithuania. Both countries are members of the European Union and the Schengen Area, so the right of free movement applies: an Italian national may enter, travel, work, study and reside in Lithuania using only a valid Italian passport or national identity card. The Schengen 90/180-day short-stay limit and the upcoming ETIAS travel authorisation apply only to visa-exempt non-EU nationals, NOT to EU citizens such as Italians. For stays beyond three months, Italians must simply register their residence with the Lithuanian Migration Department (a formality declaring an EU citizen's right of residence), but no visa or residence permit is ever required. There are no entry fees and no application process for a tourist or short visit.
